- type: ‘enterexit’ or ‘birthdate’
- required_age: an integer
- lockout_enabled: ‘1’ for yes, ‘0’ for no
- lockout_count: an integer, the number of tries a user gets
- exit_url: include ‘http://’
- form_layout: ‘standard’, ‘vertical’, ‘inline’ or ‘stacked’
- disable_date_input: ‘1’ for yes, ‘0’ for no
- disable_labels: ‘1’ for yes, ‘0’ for no
- label_enter: label on the enter button, not required if you choose ‘birthdate’ type
- label_exit: label on the exit button, not required if you choose ‘birthdate’ type
- label_birthday: label for the birthday call to action, not required if you choose ‘enterexit’ type
- label_month: label for the month field, not required if you choose ‘enterexit’ type
- label_day: label for the day field, not required if you choose ‘enterexit’ type
- label_year: label for the year field, not required if you choose ‘enterexit’ type
